Field Trip Request Form

Note to Parent:

Please enter your information into the form and then click the Submit button to register your child for his/her field trip. 

I/we are requesting permission for our child to attend the following educational field trip sponsored by Sacred Heart School.  We acknowledge the right of the school to refuse permission to take a child on a field trip because of academic, behavioral, or other reasons as outlined in the Parent-Student Handbook.

In case of an emergency, we grant permission to the school and/or their agents to render whatever aid is necessary including transportation to a hospital or emergency room for care.
